The H. Frank Carey High School Parent–Teacher Student Association (PTSA) is an all- volunteer organization that works to benefit the students at H. Frank Carey High School. Each year the PTSA funds scholarships to seniors that are chosen by the guidance department and awarded at the May Awards Night.
The PTSA is offering new scholarship opportunities for PTSA members only (or the children of members.) After careful review of all applications, seniors will be selected and awarded a $300 scholarship. Being a recipient of any award at the May Awards Night does not exclude you from these awards. The PTSA scholarship committee judges these scholarships independently.
Our scholarships recipients will be notified by May 25th and honored at the June PTSA meeting.
Your application will be reviewed and scored anonymously.
- Complete this application by typing or printing your answers as neatly as possible.
- Place Personal Information page in an unmarked, sealed envelope stapled to the front of your completed application.
- Return to the PTSA mailbox located in the main office on or before May 18, 2018.
- No late applications will be accepted.
